After some back and forth with a rogue form, I finally completed the Production Questionnaire.
Part of this was to help the in-house designer work on a cover for the book. I have a book cover mocked up and forwarded that too. They encourage you to be part of the process at all stages.
I also supplied a photo of myself for use on the website and book cover.
